Mail-In Rebate

Get a $50 Visa Debit Card on select phones when you add a qualifying data access plan to any new two-year service agreement or an $80 Visa Debit Card on smartphones with eligible voice and data plan attachment.

Follow these 4 simple steps:

  1. Purchase and activate a current model digital cellular phone and service between October 1st, 2010 - January 13th, 2011.
  2. Complete the Rebate Form.
  3. Make copies of all rebate materials for your records. Any disputed claim will require photocopies for resolution.
  4. Mail the following to the address listed below:

Note: If ordering multiple phones, you must complete a separate form for each phone and send in separate envelopes.

Terms: Rebate will be paid in the form of a U.S. Cellular Visa Debit Card. Mail-in rebate only valid with purchase of an eligible digital cellular phone and activation of an eligible service plan. Eligible handsets or wireless modems must be a least $49.95 to receive a $50 Visa Debit Card and smartphones at least $79.95 to receive an $80 Visa Debit Card. Customer's wireless service and required features must be active when the offer form is processed and validated to be eligible for the U.S. Cellular Visa Debit Card. Purchase of an eligible data plan or eligible unlimited product bundle is required to qualify for rebate (excluding prepaid, smartphones & wireless modems). An eligible data plan or eligible unlimited product bundle must be on account for minimum of 90 days. Smartphones require minimum purchase of an eligible smartphone data plan and eligible voice plan for the rebate on smartphone devices. Wireless modem requires $59.99 5GB Wireless Modem service plan. Please allow approximately 10-12 weeks for processing after claim is received by fulfillment company. Purchases must be made between October 1st, 2010 - January 13th, 2011. All rebates must be postmarked by February 14th, 2011. A separate rebate form must be completed and mailed in a separate envelope with all required documentation for each rebate submittal. Limit one rebate for each qualified device and wireless number. If more than one purchase is reflected on a receipt, make a copy of the receipt for each submission. Limit 1 (one) rebate per cell phone number within a twelve month period. Service agreement of 1 (one) or 2 (two) years is required for rebate except on Prepaid accounts. Account must be active and in good standing on a U.S. Cellular qualified service plan to qualify for rebate. Use of multiple addresses or P.O. Boxes to obtain additional rebates is considered fraudulent and may result in prosecution. (18 USC 1341 &1342). All rebates will be paid in U.S. dollars. U.S. Cellular and affiliates are not responsible for lost or misdirected mail or ineligible entries. Illegible or incomplete requests will not be honored. Please keep a copy of ALL materials submitted for your rebate. U.S. Cellular associates, agents and affiliate accounts are not eligible to participate in mail-in rebate program. Rebate not valid at Best Buy, American TV, and Corporate owned Radio Shack stores. E-mail addresses are solely for the use by USCC and will not be sold to any third parties. The U.S. Cellular Visa Debit Card is accepted anywhere Visa debit cards are accepted. The U.S. Cellular Visa Debit Card is not redeemable for cash and may not be used for cash withdrawal at any cash-dispensing locations or automated gasoline pumps. The U.S. Cellular Visa Debit Card is non-transferable and non-refundable. Cardholders are subject to terms and conditions of the card as set forth by the issuing bank. The U.S. Cellular Visa Debit Card is issued by MetaBank pursuant to a license from Visa U.S.A. Inc. Funds are available at US locations for 120 days after the U.S. Cellular Visa Debit Card issuance, through the "Expiration Date" shown on the card.
